CHC accepts most private insurance policies, Medicaid, Medicare and Utah CHIP.  Patients without insurance may qualify for a "sliding fee" discount based on income, family size and an ability to pay.  Co-payments are due the day of service unless prior arrangements have been made.  For more information concerning payment options click here.
