Ways to Give Cash Donations
Direct donations: You may send a gift directly to
Community Health Centers, Inc. or you may make an online donation.
Please see the links below if you would like to make a donation. We
accept both checks and credit cards. Many employers match gifts made by
their employees. Check with your employer to see if this is an option
for you.
Honorary, Memorial and Special Occasion gifts:
You can honor the special people who are an important part of your
life with an honorary or memorial gift. An honorary or special
occasion gift recognizes a special person or event in your life. A
memorial gift is a lasting reminder of their past presence in your
life and a remembrance of their unique contributions. If you would
like to make a memorial or honorary gift, you may download and
complete the donation form or make an online donation.
Monthly or Quarterly Giving: You may spread
commitments to Community Health Centers, Inc. over several years
and pay in annual, quarterly or monthly installments. We will
send payment reminders upon request. A pledge can make it easier
for you to give as generously as possible. For example, you can
pay a pledge of $3,000 in quarterly installments of $375 over
two years; as annual installments of $1,000 for three years; or
$750 for four years. To establish a pledge, you may download and
complete the donation form.
Planned giving: A
planned gift to Community Health Centers, Inc. supports our
work in the community. Planned gifts include:
- Wills and Estate Plans: Bequests can be a fixed
amount or a percentage or the remaining value of
your estate after loved ones are cared for.
- Life insurance: Your gift of life insurance to a
charitable organization is a gift that continues in
perpetuity. You can specify how the cash value
proceeds of the policy are used. You may also make a
gift which will pay the premiums for a policy in
your name.

Give While You Search the Web Through "Good Search"
Donate While You Search:
The idea behind this excellent search engine (powered by Yahoo) is simple and brilliant — 50% of all ad revenue generated from
the site is donated to the charity of the user's choice, and the money GoodSearch donates comes from its advertisers, so it
doesn't cost the users or the organizations a penny.
Vehicle Donations
When people generally think to donate automobiles to charity, it is with
the thought that the charity will use the vehicle to transport individuals
or supplies, but at Community Health Centers, Inc. this is not the case.
When someone chooses to donate an automobile to Community Health Centers,
Inc., it is converted to cash to provide quality healthcare for the
low-income uninsured persons from Salt Lake County. When this is done, the
money from the vehicle might be used to fund immunizations, dental care or
prenatal care programs for low-income uninsured persons from Salt Lake
County.
By giving a car, truck, SUV, or other vehicle to Community Health
Centers, Inc., the donor can deduct the fair market value from their income
taxes. Of course, the biggest reward when people make donations of any kind
and size is the knowledge that they provided assistance to someone who
needed it. Donating a vehicle is probably the largest gift many people will
ever be able to give to a cause they believe in. Most people don't have the
kind of income that will allow them to make such large donations any other
way.

In-kind Donations
CHC accepts various forms of in-kind donations. Our “Reach Out and Read”
program is in constant need of gently used or new pre-school and elementary
aged books...especially in Spanish. A book is given to a child patient at
each visit to encourage reading at home.

CHC has adopted a

Volunteering at CHC
CHC BOARD OF GOVERNORS
Community Health Centers, Inc. is governed by a volunteer Board of
Governors. The Center’s by-laws and federal program regulations require
that at least 51% of the Board be recipients of the Center’s services.

here for more information about our current
board.
WAITING ROOM READERS
Through our
"Reach Out and Read" (ROR) program, we have
readers in our waiting areas, where you would read to the pediatric patients
who are waiting to be seen. This is a national pediatric literacy program
offered in our clients. At every well-child visit, our pediatricians
encourage parents to read aloud to their children and offer age-appropriate
tips. The
"Reach Out and Read" (ROR) program is
funded in part by a grant from Target and funds from individual donors.
To volunteer,
contact the clinic director at the
CHC facility which you want to volunteer.
Grassroots Advocacy
The National Association of Community Health Centers (NACHC) has a convenient
website
supporting grassroots advocacy. NACHC is the voice for America’s community health centers and the people they are committed to serve.
Community, Migrant, Public Housing and Homeless Health Centers provide access to quality, affordable health care to anyone, regardless of their
ability to pay and serve as the family doctor and health care home for more than 15 million people. Through its successful advocacy and
communications efforts over more than 30 years, NACHC has earned broad bipartisan support among federal, state, and local policy-makers.
NACHC has worked hard to promote the mission and accomplishments of health centers and to secure the federal support and resources that
will protect and strengthen existing health centers and allow them to expand their reach to communities in need of the care they provide.
AUCH (Association for Utah Community Health) advocates for policies that will increase access to
health care for the medically underserved, including patients who are
eligible for Medicaid, Children Health Insurance Plan (CHIP), Primary Care
Network (PCN) and those who are uninsured or underinsured. AUCH also
advocates for policies that will support Community Health Centers in their
role as the medical home for the medically underserved in the state. The
AUCH website has a tool to make it easy to submit your opinion to state and
federal representative. To advocate for better healthcare,
